How to Request Records
Clients or authorized representatives may request copies of treatment records, assessments, or other documents by completing our Release of Information Form.
Please include:
The client’s full name and date of birth
The specific records or date range requested
The purpose of the request
The name, address, or fax/email of the person or organization to receive the records
A valid signature and date of authorization

Processing Time
Requests are typically processed within 10 business days of receipt. You will be notified if additional time is required.
